Details

The Assistant Director of Intake plays a critical role in ensuring a smooth and compassionate admission process for patients and their families. This position supports the Director of Intake in managing referral coordination, patient assessments, and communication between internal teams, healthcare providers, and families. The ideal candidate will have a strong background in healthcare intake, excellent communication skills, and a passion for providing quality end-of-life care.  This position would likely work  Thursday-Monday schedule. 

 

Duties include:

  1. Direct Supervisor for employees of designated team(s).  This includes but not limited to Admissions RN, and/or Referral Coordinators at the direction of the Director of Intake.
  2. Ensures adequate staffing levels and arranges for coverages as needed related to PTO or sick calls
  3. In coordination with the intake staff, supports admissions as necessary
  4. Support and mentor referral coordinators, providing guidance on processes, documentation, and best practices.
  5. Leads and responsible for management of Referral Tracker. Including updates and scheduling admissions and discharge planning appointments.
  6. Assist in overseeing the intake process, ensuring timely response to referrals from hospitals, physicians, nursing facilities, and families.
  7. Provides support to Admission Nurses with training and orientation, as well as ordering supplies and/or medications.
  8. Serve as a liaison between referral sources, clinical teams, and administrative staff to facilitate smooth transitions for patients.
  9. Work with the clinical team to review referrals, assess hospice eligibility, and ensure compliance with Medicare, Medicaid, and private insurance guidelines. Consults with Medical Director as necessary
  10. Assist in developing and implementing procedures to enhance efficiency, accuracy, and patient experience in the intake process.
  11. Ensure all required documentation is completed accurately and in a timely manner, adhering to HIPAA regulations and hospice policies.
  12. Assists in recruitment of new hires
  13. Works with Hospice Care Liaisons (HCLs) to promote positive community image and relationships
  14. Participates in daily morning huddle meeting with HCLs, leadership, and intake team
  15. Participates in Administrator On-Call rotation
  16. Responsible for compliance audits of assigned team
  17. Assists Director of Intake and other supervisory hospice personnel in the planning, implementation, and evaluation of in-service and continuing education programs.  Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range.
  18. Supports all CHAP audits
  19. Assesses health needs of community and assists in the development of plan(s)/ program(s) to meet needs.  Participates in public relation and community activities that promote Suncrest Home Health and Hospice's role as an effective member of the hospice and health care delivery system.  Promotes customer service orientation to all hospice personnel.
  20. Performs other duties and activities as delegated by the Director of Intake

Qualifications

  1. Registered nurse with current licensure to practice professional nursing in the state.
  2. Graduate of NLN accredited school of nursing with a BSN degree is preferred.
  3. Minimum of 2 years in a healthcare intake, admissions, or case management role, preferably in hospice or home health
  4. Strong leadership and team management abilities.
  5. Strong organizational skills and attention to detail.
  6. Proficiency in electronic medical records (EMR) systems.
  7. Complies with accepted professional standards and practice.
  8. Has excellent observation, good nursing judgment and communication skills.  Understands hospice philosophy and issues of death/dying.
  9. Understands principles of pain/symptom management.
  10. Knowledge of hospice regulations and insurance processes is a plus.
  11. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  12. Complies and maintains current CPR certification.
  13. Demonstrates excellent observation, verbal and written communication skills.
