Details

Position Summary:
The Float Hospice Registered Nurse (RN) provides direct patient care and clinical support across the Northbrook and Westmont service areas. This role is responsible for delivering skilled nursing care, symptom management, and end-of-life support to patients and families, while providing coverage and continuity of care across teams as needed. The Float RN will collaborate with the interdisciplinary team (IDT) to ensure patient-centered care and serve as a reliable clinical resource for both offices.

 

Key Responsibilities:

  • Perform RN comprehensive assessments and ongoing follow-up visits in accordance with hospice standards of care.
  • Provide symptom management and crisis intervention visits to support patient comfort and quality of life.
  • Conduct hospice admissions and re-admissions, ensuring timely and accurate completion of required documentation.
  • Conduct hospice patient recertification assessments to ensure continued eligibility and compliance with regulatory guidelines.
  • Respond to and complete death visits with professionalism, compassion, and adherence to regulatory guidelines.
  • Provide coverage for RN Case Managers (RNCMs) during periods of PTO or vacancy to ensure seamless patient care.
  • Participate periodically in Interdisciplinary Team (IDT) meetings to support care planning and coordination.
  • Communicate effectively with physicians, patients, families, and team members to support individualized care plans.
  • Maintain accurate, timely, and thorough documentation in the electronic medical record.
  • Support hospice standards, policies, and compliance with state, federal, and accreditation requirements.

Qualifications

  • Qualifications:

    • Current Registered Nurse (RN) license in the state of Illinois (required).
    • Minimum 1–2 years of clinical nursing experience; hospice or palliative care experience strongly preferred.
    • Strong assessment, critical thinking, and clinical decision-making skills.
    • Ability to work independently across multiple locations while maintaining collaboration with team members.
    • Compassionate communication skills and ability to support patients and families through end-of-life care.
    • Valid driver’s license, reliable transportation, and ability to travel between Northbrook and Westmont offices and patient homes.
  • Schedule:

    • Monday through Friday, 8:30am – 5:00pm
    • Ability to provide coverage for planned and unplanned PTO for RN Case Manager
